Patient Qualifications

To qualify for medical care at Guadalupe Clinic, you must meet all of the following criteria: 

  • Be without health insurance of any kind (including Medicare, Medicaid, HealthWave, or other medical assistance through DCF)
  • Have an income within 250% Federal Poverty Level (will be verified with proof of income)
  • Must not be under the care of any other physician
  • No Workman’s Comp or other insurance claims

If you meet the above criteria:

    Call Guadalupe Clinic (316-264-8974) for further information. Medical staff will then review your eligibility and schedule an appointment.

 

Once You Are Accepted

Patient’s first visit at Guadalupe Clinic: Each patient MUST bring proof of income and will be asked to donate $5 per visit; however, this is not a prerequisite for service.

POSSIBLE TYPES OF PROOF OF INCOME:
  • 30 Days of paycheck stubs
  • Letter from Unemployment Office verifying amount and duration
  • Letter from Social Security verifying you receive monthly disability income
  • Letter from whomever is supporting you stating they are currently your sole support
  • Federal income tax return
